As of 1st March 2023 to register with the practice you must be living in one of the following postal districts:

G11- 5, G11- 6, G11- 7, G12- 0, G12- 9, G13- 1, G14- 9

How to Register

You will need to complete a registration form and a new patient information questionnaire. You can either download the forms, complete and submit our online registration form, or collect them from reception. If you are collecting the forms please complete and return these to the surgery. If you already have a GP in the local area you will be asked to make an appointment with a GP partner prior to registration.

Acceptance on to the list is as at the discretion of the Doctor and we will ask you for proof of identity. (Please refer to our registration policy below and why we ask for this). Please note that your medical records may take several weeks to arrive at the Practice.

If you would like a basic health screen on joining the practice please feel free to make an appointment with the Practice Nurse. If you have ongoing health problems such as diabetes, breathing problems, heart disease, epilepsy, and blood pressure or stroke or are on regular medications we would ask you to make an appointment with one of our doctors after registering.

Ethnicity Recording

This practice, in line with other healthcare providers, collects information about the ethnic group of patients. This information can help us plan to meet the needs of the community and ensure that everyone has equal access to the health care we provide.

Please note we are not asking about citizenship or nationality, but about the ethnic group to which you feel you belong.

All the information we receive will be used and treated with the strictest confidence. Any planning information on general release will be anonymous with all names removed. The classification is entirely voluntary but will help us to provide a better service.

There are a number of reasons why you may not be able to register with your chosen GP, for example you may live too far away. If this is the case simply choose another GP in your local area.

It is is important to note that you can only permanently register with a GP practice if you’re staying in the area for more than 3 months.

PLEASE COMPLETE STEPS 1 AND 2 BELOW AS YOU WILL NOT BE REGISTERED UNTIL WE RECEIVE BOTH FORMS.

Step 1: Download registration form, via NHS inform and upload to the practice on the form below. If you are unable to do this please contact the surgery for advice on how to register.

Step 2: Please complete our New Patient Questionnaire, once completed please upload along with the New Registration Form.

Once you have completed the forms please upload the forms to the practice. Please note if you do not complete both Steps this may delay your registration. Please upload to the practice a completed New Patient Questionnaire, and copies of two forms of your identity.
One form of identity must contain a photo of yourself and the other must provide evidence of your current address. You may upload your forms online.

Alternatively you can attend the practice and fill out forms at reception.
